All Monsters & Anomalies in Animal Hospital
Surviving the night shift comes down to one rule: every threat has exactly one correct counter. Weapons only work on the Skinwalker — every other monster is handled by a specific item or action, and using the wrong response usually makes things worse or kills a patient. Below is the full bestiary, split into enemies that roam the clinic and anomalies you must spot at the check-in desk.

Enemies & environmental hazards
These appear inside medical rooms, hallways, and corners during your shift. They are separate from patient anomalies and cannot be rejected at the front desk.
Skinwalker
A tall black rabbit-like figure with oversized eyes, an unnaturally wide grin, and a white outline that makes it visible even at the edge of vision. It first appears at reception disguised as a normal patient — only the CCTV feed reveals its true form. Once admitted, it can awaken at any time, in any medical room, and immediately hunts patients or you.
Counter: The Skinwalker is the only enemy you fight with a weapon. Approach with a taser or gun and defeat it with M1 attacks. If you have no weapon and it is attacking a patient, hold E from behind to take it down cleanly. If it is on you directly, press E repeatedly to break free — you will lose sanity in the process.
Prevention is better: the cleanest fix is to never let one spawn. Catch anomalies at the desk using visual, camera, and photo checks, and hit the shutter button before they walk in. Bed Monster
A long black rabbit-like creature with a disturbing smile that spawns beneath the beds in medical rooms. You will see a red warning line on the floor in front of the bed and a pair of giant black hands beside it. Looking directly at the red eyes underneath drains your sanity. It can attack a patient while they are lying down, walking in, or walking out.
Counter: Equip Maple Syrup and walk up to it — the monster takes the syrup and disappears without harming anyone. Approach without syrup and it grabs you, draining sanity. If it is already dragging a patient under the bed, get close and interact with the patient to help them break free. You have 30 seconds before it eats them. The Bed Monster cannot be permanently defeated, only handled item-by-item.
Wall Banger (Head Knocker)
A patient with sunken, hollowed-out eyes (called Head Banger or Head Knocker in some guides) that walks up to the reception window and repeatedly bashes its head against the glass. While the Wall Banger is banging, the shutter button is disabled — so any anomaly patient walking in during that window cannot be rejected. That is the real danger: not the Wall Banger itself, but the Skinwalker that slips through while you cannot close the shutters.
Counter: Approach with coffee or food and interact to remove the Wall Banger calmly. You can also hold E to ask it to leave with no item, but doing so costs a large amount of sanity. Keep coffee stocked so you never have to take that hit.
Don't Look Up (Flesh Ceiling)
A blob of dripping flesh studded with multiple eyes that clings to the ceiling of medical rooms. Also called the Flesh Ceiling in some guides. The moment you enter a room that has one, the game flashes a "Don't Look Up" warning on screen. If you look up at it, the flesh stares back and drains your sanity continuously. The longer you look, the more it grows.
Counter: Keep your camera level, finish whatever task you are doing, and it eventually disappears on its own. Look up at it and the sanity drain begins immediately. Want it gone faster? Hold Eye Drops, open the door inward, jump on top of the door, then slowly look up — the treatment prompt removes it for good.
Stalker
A tall, dark bunny-like silhouette with an exaggerated grin that spawns randomly around hallway corners. It does not chase you and does not attack patients — its entire threat is direct eye contact. While it is in your camera view, sanity drains at roughly 10% per stare.
Counter: Spot it from a distance, then keep your camera angled down or away as you walk past. Tap your mouse to break line of sight — do not stare. Weapons do nothing here, so do not waste taser cooldowns. The Stalker disappears behind the corner it came from after a short while.
New behavior (Harlow's Favorite Update, July 10): the Stalker can now also appear at the check-in window. When it does, craning its body to look through the glass, take its photo — that interaction makes it vanish. A few steps from the office is also a high-spawn zone, so do not linger staring down any corridor.
Wall Anomaly
Small black creatures that appear on walls, chase you, and steal your items. They drain sanity on contact.
Counter: Aggro it, then run away — it disappears into black smoke after a while. There is no item that defeats it, so mobility is the only answer.
Surgery Tentacles
Tentacles that appear in Room 8 during surgery. They are a hazard layered on top of the timed surgery mini-game.
Counter: Avoid them and finish the surgery mini-game. Never shoot or taze them — doing so kills the patient automatically. See the treatment rooms guide for the full surgery walkthrough.
Green Sludge / Slime
An environmental terrain hazard that blocks your path through the clinic.
Counter: Hold E to clear it. No item required, no sanity cost — just don't walk through it.
Check-in anomalies (patient signs)
These are the signs that a patient at the reception desk is an anomaly, not a real animal. Run all three checks on every patient: visual inspection at the window, the security camera feed, and a photo. If anything fails, hit the shutter and reject them.
The patient's ears look different on the camera compared to what you see at the window. Pay close attention to ear shape and size.
A figure appears when you check a broken CCTV feed. Exit the camera quickly or take a major sanity hit from a jump scare.
A special NPC who asks you to keep a suitcase safe. Reject his request — giving him coffee then taking his photo confirms he is not what he seems.
The game ships with more than 14 ways a patient can be "wrong." When in doubt, close the shutters — rejecting a real patient by mistake is far better than letting an anomaly through.
