How to Play Animal Hospital (Anomaly)
Animal Hospital is a co-op survival horror game on Roblox where you work the night shift at an animal hospital. Some patients arriving at your reception desk are not what they appear to be. Your job is simple to describe and hard to execute: identify the real patients, treat them in the correct room, and reject the anomalies before they turn into Skinwalkers and hunt the clinic.
The core loop
Every shift follows the same rhythm:
- 1Check in a patient. A new animal arrives at the reception desk. Run your three checks before letting them through.
- 2Decide: real or anomaly? If anything fails a check, hit the shutter and reject them. If they pass, assign them to a treatment room.
- 3Treat the patient. Go to the room and complete its mini-game, then administer the correct items.
- 4Handle interruptions. Events and emergencies pop up on the right side of the screen. Monsters may also appear in rooms and hallways — counter each one correctly.
Do this consistently while keeping your sanity above zero, and you clear the shift.
The three checks (most important skill)
Every patient must pass three checks before you admit them. The game ships with 14+ ways a patient can be "wrong," so never trust a patient that looks normal at first glance.
Look at the patient through the check-in window. Check ears, posture, expression — anything off is a red flag.
Switch to the security camera feed. Some anomalies only show up here, not at the window (for example, different ears).
Always take a patient's photo before registering them. Photo checks join from the middle shifts onward.
Golden rule: when in doubt, close the shutters. Rejecting a real patient by mistake is far better than letting an anomaly through — a rejected real patient costs you a little, but an admitted anomaly becomes a Skinwalker that hunts the whole clinic.
Treating patients
Real patients get assigned to one of the treatment rooms. Each room has its own mini-game and timing rules. See the full treatment rooms guide for step-by-step walkthroughs.
- • You can step away from Basic Medical, X-Ray, and Heart Monitor mid-task and return later.
- • Surgery is the exception — once it begins, stay until it is done or the patient dies.
- • A red line above a room number means a patient is inside — use it to navigate fast.
Beginner survival rules
Sanity is the only number that ends your run. Restock whenever the shop is convenient.
Using the wrong response wastes sanity or kills a patient. Learn the counter table before shift 4.
Don't wait until something feels wrong — routine camera sweeps catch anomalies the window misses.
Emergencies force you to move between rooms. Never get caught with low sanity and no buffer.
Weapons only work on Skinwalkers. Don't waste ammo on monsters that need an item counter.
